Rates

Child/youth autism spectrum evaluation (ages 3+): $2215

Adult autism spectrum evaluation (ages 19+): $2425

Child ADHD evaluation (ages 3-18): $2150

Adult ADHD evaluation (ages 19+): $2250

Learning disability/differences evaluations: $200/hour, total variable depending on question

Insurance

I am not paneled with any form of medical assistance (e.g., Badgercare, Medicaid). I am currently paneled with Group Health Cooperative of South Central Wisconsin (GHC-SCW). GHC-SCW requires that you obtain a prior authorization through your primary care provider or therapist before seeing me.

Payment

I accept cash, check and all major credit cards as forms of payment, including health savings accounts.
